Exercise plus nutrition,
does it really help with Reduced incident mobility disability defined by failure of the 400-m walk?
research showsThe grade is B with 70 points. In SPRINTT's prespecified primary population of 1,205 participants, mobility disability occurred in 283/605 (46.8%) versus 316/600 (52.7%), HR 0.78 (95% CI 0.67 to 0.92).

Useful facts when choosing a product
- The program combined aerobic, strength, flexibility, and balance exercise with individualized energy and protein goals.
- Controls received healthy-aging education and brief upper-extremity stretching.
- The registration is NCT02582138.
What the research actually shows
SPRINTT randomized 1,519 participants at 16 sites in 11 European countries and prespecified 1,205 participants with SPPB 3-7 for the primary efficacy comparison. The article reports a web-based permuted-block system, masked outcome assessors, intention-to-treat analysis, a predefined analysis plan, and concordance with the registered primary outcome. Participant masking was infeasible, but walking assessment was masked and missed tests used a predefined algorithm and independent committee. Funding combined a public-private initiative with industry in-kind support. Verdict 2593 (B, 70 points) tested long-term structured exercise alone for the same outcome; this verdict tests exercise plus nutritional counseling.
Why this is classified as B (70)
A large prespecified trial improved a patient-centered mobility outcome without an identified avoidable major flaw, but absent independent replication gives B with 70 points.
Counterpoint. Exercise intensity requires individualized fall, cardiopulmonary, and musculoskeletal assessment, and the result should not be extended to the SPPB 8-9 group.

| Endpoint | P | Patient-reported treatment goal - the symptom is the goal |
| Replication | R1 | Single confirmatory trial |
| Independence | I1 | Mixed funding sources |
| Effect size | E+ | Meets the clinically important threshold |
The scoring table and the verdict agree (B).
Sub-claim grades by effect
This ingredient is marketed for several effects. A single overall grade blends strong and weak claims together, so each effect is graded separately here. The overall grade reflects the strongest disconfirming or core claim.
| Effect (sub-claim) | Grade | Basis |
|---|---|---|
| Reduced mobility disability in frail older adults with baseline SPPB 3-7 | B | The HR was 0.78 (0.67 to 0.92). |
