What the research actually shows
SELECT randomized 17,604 participants, 8,803 versus 8,801, in a double-blind placebo-controlled event-driven trial. Diabetes, screening HbA1c at least 6.5%, and recent glucose-lowering therapy were excluded; established cardiovascular disease and overweight or obesity were required. The primary analysis used a treatment-policy Cox estimand for time to first event regardless of treatment discontinuation or background medication changes. Novo Nordisk designed and sponsored the trial and company employees were among the authors. SUSTAIN-6 was also designed and sponsored by Novo Nordisk and enrolled patients with diabetes, so it was not counted as independent same-indication replication. Verdict 841 is B with 79 points for long-term weight loss and maintenance, a different endpoint from cardiovascular event prevention here. Verdict 1587 is B with 79 points for weekly 1.0 mg in chronic kidney disease with type 2 diabetes and a kidney-failure, major kidney-function-loss, or kidney/cardiovascular-death composite. Verdict 1593 is C with 59 points for weekly 2.4 mg in obesity with moderate knee osteoarthritis and pain and physical function. Their populations, dose, or clinical endpoints differ from SELECT.
