Low-force electronic bidet,
does it really help with Noninferior relief of pain seven days after hemorrhoidectomy as a substitute for a warm sitz bath?
research showsThe grade is D. BIDLOW used an active comparator—an approximately 38-degree-C warm sitz bath—not no treatment. Day-7 post-use pain VAS was 38.3+/-21.9 with the bidet versus 42.0+/-21.1 with sitz bath, P=.453, but prespecified noninferiority was not demonstrated. A nonsignificant difference is not proof that the bidet works as well as a sitz bath.
ads claimKorea's widespread bidet use makes replacement of sitz baths a practical question. The tested device was a purpose-built very-low-force fountain system, not ordinary high-pressure washing. Postoperative wound and bleeding instructions from the surgeon take priority.
Useful facts when choosing a product
- Both groups used approximately 38-degree-C water for 3-5 minutes each morning for seven days, with optional additional use for four weeks.
- The comparator was active warm sitz bathing, with no untreated arm.
- The tested Coway Clinic Bidet used a lower-force fountain stream than commercial electronic bidets.
What the research actually shows
Four hospitals used an independent web randomization system and kept surgeons blinded. Participant blinding was inherently impractical and was not counted by itself. Three avoidable defects remained: a noninferiority design, fewer than 200 participants, and 25.7% post-randomization loss. Coway supported the study through the SNUH Clinical Research Institute and provided material and technical support; the principal investigator and Coway R&D jointly developed and patented the device.
Why this is classified as D (28)
A single manufacturer-supported noninferiority trial missed its main hypothesis and also had a small sample and 25.7% attrition, giving D with 28 points. Its margin came from applying 10% to the observed control mean and was not a patient-anchor-validated minimum clinically important difference.
Counterpoint. Wound healing was similar and convenience higher, but neither establishes pain noninferiority.
Rejudgment record. Cross-check applied — Failed active-control noninferiority primary endpoint in a manufacturer-supported small trial with substantial attrition
| Endpoint | P | Patient-reported treatment goal - the symptom is the goal |
| Replication | R1 | Single confirmatory trial |
| Independence | I0 | Evidence comes only from manufacturer studies |
| Effect size | E0 | Null |
| Precision | C0 | The confidence interval leaves room for benefit |
The scoring table and the verdict agree (D).
Sub-claim grades by effect
This ingredient is marketed for several effects. A single overall grade blends strong and weak claims together, so each effect is graded separately here. The overall grade reflects the strongest disconfirming or core claim.
| Effect (sub-claim) | Grade | Basis |
|---|---|---|
| Noninferior day-7 pain relief versus warm sitz bath | D | Means were similar, but the prespecified noninferiority test failed. |
| Wound healing at four weeks | C | Rates were 97.1% versus 97.6%, a secondary endpoint in a small trial. |
| Convenience of postoperative care | C | Convenience was higher with the bidet, 85.3% versus 56.1%. |
